Admissions

The hospital admission process is an important factor in providing you with the best care possible during your stay with us. During the admission process, you will be asked to sign medical financial consent forms. Please bring your Medicaid, Medicare, and insurance information and a photo I.D. with you at each visit. A copy of your cards will be made to expedite billing procedures. Also, please bring your living will or advance directive with you so a copy can be made for your medical record.

If you are pregnant and being admitted to have your baby, you should have received admission forms to complete before your arrival. Please bring these forms with you.
